SUMMARY - This veteran entered this facility on 1-3-67 with chief complaint of multiple bod y contusions and abrasions. He also had a fairly sever chronic brain syndrome, secondary t o alcoholism and the sclerotic changes of the vessels in his brain. He also has bilateral d eafness, severe. Apparently this man has lived alone in a one-roomed apartment in Vinita, Ok lahoma, became inebriated and walked into a switch engine and received the above mentioned co ntusions and abrasions and was brought here by ambulance from Vinita hospital. His EKG showe d wave changes compatible with pulmonary emphysema. X - ray of his chest showed the presenc e of calcific plaque in the aortic knob; lung fields were clear. X - ray of his right should er, skull and facial bones revealed on evidence of fracture. No showed some secondary anemi a in his hemogram, which revealed 3.9 RBC with 35% hematocrit a 11.5 grams hemoglobin. He ha d a BUN of 15 and his urinalysis could be considered negative. His chief medication while he re consisted of Tetracycline, 250 mg. Four times daily; Cortisporin Ointment to the scalp tw ice daily after shampoo with Phisohex twice weekly. At present time, the patient seems ver y cooperative. He still is totally deaf. Because of extraneous living conditions of this ma n, and with no relatives, he is to be transferred this date to the Waldorf Nursing Home for t rial visit. He is incompetent according to this facility's judgment; may continue his activi ties commensurate with a nursing home and no medication is being authorized at this time. Mrs. Helen Lee Foster, Social Services, Veterans Hospital, Muskogee, Oklahoma, Re: Ma rtin, Carl E., Age, 78 years. Dear. Mrs. Foster: This is a follow-up of our telephone convers ation of today regarding Mr. Carl E. Martin. The limited information was obtained throug h a telephone conversation with Mrs. L.J. Roberts, landlord of your patient. Mr. Martin ha s been renting a furnished apartment from Mrs. Roberts for the past seven years. Until thre e or four years ago he was a desirable occupant, but since this time he has become a wino an d will drink one to two fifths of wine a day; consequently he is filthy, spits tobacco, nois y and disturbs the other renters until none can sleep. She has found as many as fifteen win e bottles at one time in his room. Although his clothing and personal possessions remain the re she would not like for him to return, but she was interested in his progress. She believe s he needs to be in a Veterans Home for there are no living relatives. She said that his for mer landlady had told her that fifteen of twenty years ago he had a brother, but did not kno w if he was living. She believes that the report of a nephew in Arkansas is in error. Mr. M artin told her he did not receive his veterans or social security check for this month. Sh e said his social security check had been $55.00 before the last raise, but she did not kno w the amount of the veterans payment. He is in debt all over town and owes Mrs. Roberts $11. 00 rent. On January 1, 967, Mr. Roberts [Martin] walked into a swith engine. Luckily the en gine merely glanced him and he fell in the opposite direction, and according to Dr. O.W. Deha rt was not seriously injured. On Jan. 3, 1967 he was sent from Craig Genearal Hospital to yo ur Hospital.

Social Data Dictated: 2-7-67. On 2-3-67 the social worker saw Mrs. Grandstaff, R.W., Waldor f Senior Citizens Home, Muskogee. Mrs. Grandstaff reported the veteran to be getting along a ll right. He had received his clothing from a Mra. Alpha Rogers, Vinita, Oklahoma. He woul d not accept the blackboard and slate for communications, but he enjoyed erasing what someon e else has written on the blackboard. He will stop and let them write notes and is quite a c heerful person, seeming to enjoy kidding a great deal. She reported his disabilities were n ot hindering him from making friends in the nursing home. The social worker later talked wit h Mrs. Tressel about another situation and during this time this veteran came up and playfull y took one of Mrs. Tressel's lighted cigarettes from the ash tray. According to Mrs. Tresse l he has no living relatives. Mrs. Alpha Rogers, 619 N. 2nd Street, Vinita, Oklahoma, whos e deceased husband was a close friend of the veteran's had written, sending the veteran's soc ial security check to the nursing home. In a letter she mentioned the veteran had no livin g relatives and the tone of her letter was warm and considerate of the veteran with the expla nation she could not care for him because of her own infirmities. The matter of how to depos it his social security check to his account at VAH, Muskogee was cleared and Mrs. Tressel wil l get the check and have it endorsed by the veteran and get it to the Registrar's office at t he hospital for deposit. Social Data Dictated; 7-20-67. Carl E. Martin, a 78 year old NSC veteran, who has resided a t the Waldorf Senior Citizens Home since his placement there under VA contract on 1-24-67, wa s placed in the Maple Street Nursing Home, 927 Maple Street, Telephone No. 7-7770, Muskogee , contract was to have expired over the weekend, 7-23-67. Mr. Jim Gibso, First National Ban k and Turst Co., Muskogee, guardian recently appointed, was advised of this move on 7-20-67 , by telephone. Mr. Gibson requested the office of the Chief Attorney contact and advise hi m regarding how much and for what he is to pay. Mr. Crockett, OCA, was also advised of mov e on this date, and was asked to contact Mr. Gibson, which he said he would do. Martin died Intestate on July 14th, 1971, and as far as we know without issue, lea ving a small estate in our hands, as his guardian and under there circumstances Statutes of O klahoma require the Guardian to distribute the Estate in the same manner as though appointe d Administrator. For this purpose we have employed an attorney who will handle the legal pro ceedings for the minimun fee specified by the Oklahoma Bar Associatio in Summary Administrati on cases. The persons whose names are on the attached sheet are the only ones of whom we hav e knowledge who are entitled to share in the distribution. At this time we do not know wha t amount, if any, will be left for distribution after payment of funeral expenses of Carl E . Martin and Administration costs but it will not exceed a few hundred dollars. !BIO: [Information, written by Mary Thelma (Martin) Moore Thomas about her Uncle, Carl E. Mar tin.] Carl Martin left home as a young man and no one in the family ever heard from him again . After his death it was learned from the lady who owned the rest home, in which he had resi ded his last five years, that he had been living in Casper, Wyoming until in his latter year s he had decided to return to Oklahoma. He still thought he had no brothers or sisters livin g. The lady owner of the rest home would sometime ask him if he had someone living but he wo uld just say no. He said he had never been married. My father [Orlando Martin] preceded hi m in death by three months although he was 15 years older.

!LETTER: [Letter writen by Elston Claude Martin to his sister, Mary Thelma (Martin) Moore Tho mas - letter (in/part) reads as follows: In Lordsburg Aug 22, 1971 Dear Mary, You will soo n get a letter from a Mr. Gibson in Miskogee, who had been appointed Guardian of Uncle Carl . If you recall, Uncle Carl had been missing since in the 1920's. It seems he had been livi ng in Wyoming all these years until he went to Muskogee in 1965. He was in the V.A. Hospita l from time to time with bad heart troubles. He died of heart trouble on July 14, 1971 at ag e 82. When out of the hospital he lived with a woman who kept a few old people. I met her , she said he was totally deaf, caused from shell explosions in World War I. He got a Govt . pension. When he went into the army W.W.I., he gave Daddy and Hermans name as his relatives . When he died the V.A. wrote to O.L. Martin Hanna. Lucy Simns sent the letter on to me. A nyway I met the woman where Uncle Carl had stayed the past 5 years, when he wasn't in the V.A . Hospital. She said uncle Carl was a good man and that I looked, walked and was built lik e Uncle Carl. She said she would write notes to Uncle Carl asking if he had any relatives . All he would say was "Na Na."...
